The multidisciplinary committee of experts, under the National Pharmaceutical Pricing Authority (NPPA), has said that it would desire to get a clarification of approval of license status from the manufacturers and markets of antifungal medication clotrimazole 1% mouth paint, in connection with the fixation of the ceiling price for the drug under the regulation.

This comes after it has declared that it is a scheduled formulation based on the Drugs (Price Control) Order, 2013.
The decision was taken while considering a representation of Zydus Healthcare Ltd related to the ceiling price fixation of the mount paint formulation, after the NPPA meeting referred the matter to the multidisciplinary committee for examination.

The committee, in its meeting held on January 24, 2022, examined the company’s representation and noted that the clotrimazole 1% drops, clotrimazole 1% cream and clotrimazole 1% passerby are scheduled formulations, and a ceiling price has been given for clotrimazole 1% lotion also.
According to the DPCO explanation, it has provisions to include the formulations which have no significant differences in terms of pharmacokinetics (PK) or pharmacodynamics (PD) under the same schedule, based on which the Committee earlier declared the drug as scheduled formulation Kunder the DPCO, 2013.
However, the company said that the PK and PD or efficacy profile of any mouth paint including clotrimazole 1% mouth paint, is significantly different from that of a cream including clotrimazole 1% cream, or for that matter, a drop, including clotrimazole 1%.

"The company may be requested to provide published literature on significant differences in PK or PD of clotrimazole 1% mouth paint viz a viz other formulations that leads to different outcomes,” said the committee.
It further noted that clotrimazole is a very old drug and the formulation clotrimazole 1% mouth paint has been in the market for a long time.
“Accordingly, the Committee desired that clarification of approval of license status may be obtained from the manufacturer(s)/marketer(s) of clotrimazole 1% mouth paint for further deliberation in the matter,” concluded the expert committee.
The Committee, in a meeting held on October 11, deliberated the matter of whether the particular strength of the medicine would fall under the Scheduled formulation. The Committee decided that clotrimazole 1 percent mouth paint is a scheduled formulation based on the Explanation I of DPCO, 2013.
The explanation in the DPCO, 2013 is that any dosage form of a medicine, other than the dosage form included in the Schedule, but in same strength and route of administration, which does not have significant difference in terms of pharmacokinetics or pharmacodynamics or efficacy-safety profile over the dosage form mentioned in the list shall be considered as included.

“To elaborate, if a tablet is included, other dosage forms like conventional tablets and capsules are considered as included. However, such different dosage forms should be considered differently for purposes such as procurement policy, pricing etc.,” said the DPCO 2013.
This principle also applies to all other dosage forms, for example, oral liquid dosage forms, injectables, topical dosage forms etc., it added.
Clotrimazole pessary 100 mg has been listed in the National List of Essential Medicines (NLEM) 2015, for Primary, Secondary and Tertiary healthcare levels.
The mouth paint is used for treatment of fungal infections in the mouth and other fungal infections. The medicine works by binding with the inner surface of the mouth and will be gradually released into the saliva, later killing the fungi.
According to online medical stores, companies including Mankind Pharmaceuticals and Dabur India are selling clotrimazole 1 per cent in the market at present.

The Central Drugs Standard Control Organization (CDSCO) is the Central Drug Authority for discharging functions assigned to the Central Government under the Drugs and Cosmetics Act.
CDSCO has six zonal offices, four sub-zonal offices, 13 port offices and seven laboratories under its control.
Post : Junior Scientific Assistant
Applications are invited from eligible candidates to fill up One post of Junior Scientific Assistant, Group B Non Gazetted, in PB-2 , Rs.9300-34800 with Grade pay of Rs 4200 Pre-revised, in the pay Matrix Level-6 Rs.35,400/- 1,12,400/ revised, plus allowances as per Central Govt.

Dear Readers, with the beginning of the first edition this year, I shall take up a new subject in pharmaceutics. This is my expertise as well.
I shall briefly mention one by one the topics which are studied under this subject in different years of under graduation. B. Pharmacy 1st year has a subject called Pharmaceutics-I.
Starting with the subject, knowledge about pharmacopeia is given. What information do they contain? How did they evolve? The Indian Pharmacopeia is an official book.
It is a collection of legal documents established by a national or regional body concerning standards and quality criteria for medications used in the country or region.
It consists of suitable tests that confirm the identity and purity of the product and determine its strength.
Dosage forms
Dosage forms: An introduction to the various dosage forms is given. Dosage forms are the carriers thru which drugs are delivered to sites of action within the body.
Students learn about the different dosage forms existing in the market such as tablets, capsules, pills, dusting powders, granules.
Liquid dosage forms: Liquid dosage form can be monophasic or biphasic.
Monophasic: It includes syrups, solutions, liniments, lotions, mouthwashes, gargles, inhalations.
Biphasic: It suspensions and emulsions. Semisolids dosage form include mostly external use preparations such as ointments, creams and pastes.
They learn their formulation, packaging and labeling as well in the practical classes.
Posology: Another important topic learnt in these basics is posology. It is defined as the branch of science that deals with the dose or quantity of drugs that can be administered to a patient to achieve the desired therapeutic effect.
The different factors affecting the dose of patient are age, sex, body weight, route and time of administration.
Other factors which also influence drug effects are environmental factors, emotional factors, changes in metabolic disturbances, presence of other diseases.
Drugs may have additive effects, synergism, antagonism, tolerance and interactions between them or with food as well.
We shall proceed with other topics in pharmaceutics—I in successive chapters.

Analytical chemistry deals with various analytical procedures to study different characteristics of drug. It deals with scientific and technical research on the creation of new drugs including methods of synthesis, results of pharmacological, biochemical studies.
This is done by various analytical techniques. These help in investigation of structure activity relationships in a compound which serves as guide for prediction of new compounds.
This includes techniques such as titration (acid-base, iodometric, complexometric), flame photometry, atomic absorption spectroscopy and many more.
Higher-level classes cover more advanced techniques such as ultraviolet spectroscopy, infrared spectroscopy, mass spectrometry, and nuclear magnetic resonance.
Earlier, this subject was in 2nd year of the course, but as per new syllabus, it starts from 1st year.
B. Pharm 1st semester syllabus starts with basics such primary and secondary standards used for analysis.
Their formulation, method of preparation and properties. Various titration methods , indicators used are also studied including acid base titration, iodometric titration, redox titration, complexometric titration and precipitation titration.
Electrochemical methods of analysis such as potentiometry, polarography and conductometry are also learnt.
2nd semester, 3rd semester and 4th semester chemistry is based on organic compounds, their synthesis, structure, classification, nomenclature, properties and reactions. Different functional groups and their properties are taught.
Practical syllabus includes identification of compounds and functional groups associated with it.
This forms the basis of medicinal chemistry where student learns about the synthesis of drugs. They also synthesize the same in practical classes.
7th semester students study various methods of Instrumental Analysis. It deals with the application of instrumental methods in qualitative and quantitative analysis of drugs.
The course is designed to impart a fundamental knowledge on the principles and instrumentation of various spectroscopic techniques.
It focuses on both theoretical and practical understanding of contemporary analytical tools used in drug testing.

Analytical chemistry deals with various analytical procedures to study different characteristics of drug. It deals with scientific and technical research on the creation of new drugs including methods of synthesis, results of pharmacological, biochemical studies.
This is done by various analytical techniques. These help in investigation of structure activity relationships in a compound which serves as guide for prediction of new compounds.
This includes techniques such as titration (acid-base, iodometric, complexometric), flame photometry, atomic absorption spectroscopy and many more.
Higher-level classes cover more advanced techniques such as ultraviolet spectroscopy, infrared spectroscopy, mass spectrometry, and nuclear magnetic resonance.
Earlier, this subject was in 2nd year of the course, but as per new syllabus, it starts from 1st year.
B. Pharm 1st semester syllabus starts with basics such primary and secondary standards used for analysis.
Their formulation, method of preparation and properties. Various titration methods , indicators used are also studied including acid base titration, iodometric titration, redox titration, complexometric titration and precipitation titration.
Electrochemical methods of analysis such as potentiometry, polarography and conductometry are also learnt.

Beside the different topics discussed in the previous chapter, the Subject of Pharmaceutics-I in the first semester deals with prescriptions.
A prescription is written by a medico (physician, dentist, or any other medical practitioner) and is meant for dispensing a specific medication to an individual patient.
Prescription
It consists of the instructions for the pharmacist in case of compounding medicine. Such instructions are obsolete now as almost all the medicines are ready to be dispensed in the allopathic system of medicine.
Prescription also contains the commands or information regarding the mode, time, and frequency of medication. Any special instructions or precautions regarding drug-drug interactions or food-drug interactions are also mentioned.

A student is also given knowledge about pharmaceutical calculations. An understanding of different weight systems is a prerequisite prior to these calculations.
There exist two systems of measurement.
- Imperial system
- Metric system
Imperial system: It further consists of Apothecaries system with pound as standard unit and avoirdupois system which has grain as standard unit.
Imperial system is however not in practice. Calculation methods are taught and students are trained in practical classes as well.
Solid dosage: These forms, including single doses (tablets, capsules), and multidoses (powders, insufflations), are covered in the syllabus.
Solid dosage forms meant to be inserted into body cavities such as suppositories (rectal, vaginal, uretheral, nasal ) are also covered and formulated in the laboratory.
Formulation and preparation of various monophasic liquid dosage forms, including internal (syrups, elixirs) and external (lotions, liniments, paints) are studied in detail.
Similarly, biphasic dosage forms such as suspensions and emulsions are taught. Emulsions are biphasic dosage forms stabilized by emulsifying agents.
Emulsifiers or emulsifying agents is a big class of excipients with varied properties and added as per the requirement of final product.
A basic understanding of various dosage forms is really wonderful for a student who steps into the field of pharmacy.
Pharmaceutics subjects taught in higher classes shall be dealt in next chapter.
